Health Care Navigation

  • Monitor medication regimen, vital signs and provide nursing assessment and intervention as needed in communication with the client’s medical providers.
  • Accompaniment and transportation to medical appointments.
  • Integrate and interpret health information and services across disciplines.
  • Act as a liaison between the health care team, the client and family to avoid crisis and achieve an optimum level of health and independence.
  • Coordinate hospital visits and discharge care.
  • Discuss advance directives, and refer to an elder law attorney if needed.
  • Discuss end-of-life care when appropriate , and refer to hospice when appropriate.

Psychosocial Services

  • Facilitate family meetings to discuss goals, concerns and needs.
  • Emotional support for client and caregivers.

Advocacy and In-home Support

  • Assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) such as bathing, dressing, meals, light household tasks and more.
  • Prepare meals in your home or arrange to have meals delivered to you.
  • Perform safety assessment during each visit.
  • Assist with finding reliable help with lawn service, handymen and other contractors, and coordinating moving.

Education and Referrals

  • Elder law attorneys.
  • Occupational therapy and physical therapy.
  • Medical and geriatric specialists.
  • Support groups and counseling support.
  • Emergency response systems.
  • Funeral planning.
